EVALUATION:
Wound care:
1. Assist patient into a comfortable position with easy access to wound area
2. Carefully remove old dressing using small amount of sterile normal saline solution, if necessary
3. Note presence , amount , type, color, odor, of any drainage on the dressing and inspect wound site fro size drainage and appearance.
4. Put on sterile gloves
5. Clean the wound with sterile cleaning solution (top bottom, center outside)
6. Dry the area using sterile gauze sponge
7. Apply layers of dressing over the wound (1s, 2nd and 3rd outer layer)
8. Remove and discard gloves , secure dressing with plaster
9. Label dressing and remove all remaining equipment
